Abstrak

Latar Belakang. Kanker prostat yang telah mengalami metastasis (terutama di tulang) sering tidak terdiagnosis dengan baik dan memiliki prognosis yang kurang baik. Sehingga, sangatlah penting dilakukan pemeriksaan dini dalam mendeteksi metastasis. Indikator pemeriksaan sidik tulang yang sering dipakai adalah kadar prostate specific antigen (PSA) dan gleason score (GS). Penelitian ini bertujuan untuk menganalisis seberapa kuat faktor-faktor tersebut dalam mempengaruhi kejadian metastasis tulang pada kanker prostat

Subjek dan metode. Penelitian retrospektif analistik dengan rancangan potong lintang pada 189 data rekam medis pasien kanker prostat yang menjalani pemeriksaan sidik tulang Tc-99m MDP di RSUP Dr. Hasan Sadikin periode januari 2012 sampai desember 2016. Data PSA, GS dan gleason grade (GG) dianalisa secara bivariate dan multivariate.

Hasil. Dari 189 data rekam medis hanya 110 rekam medis yang masuk dalam kriteria inklusi. Dari hasil analisa bivariate, hanya PSA (p = < 0,001) yang memiliki asosiasi bermakna dengan kejadian metastasis tulang, namun semakin tinggi derajat GS maupun GG maka kemungkinan metastasisnya semakin tinggi (GS p= 0,054 dan p= 0,036). Dari analisa multivariate, PSA tetap menjadi faktor yang paling kuat asosiasinya dengan kejadian metastasis tulang pada kanker prostat dengan nilai p = 0.001.

Simpulan. Dari analisa bivariate dan multivariate, kadar serum PSA memiliki asosiasi terhadap kejadian metastasis tulang pada pasien kanker prostat. Pada klasifikasi berdasarkan GS dan GG, semakin tinggi GS ataupun GG semakin tinggi pula kejadian metastasis tulangnya. Sehingga, hasil tersebut dapat membantu dalam mengidentifikasi pasien kanker prostat yang memiliki risiko tinggi untuk terjadinya metastasis tulang.
