Treatment options for unresectable hepatocellular carcinoma

Hepatocellular carcinoma (HCC) merupakan kanker hati primer yang terjadi akibat tumbuhnya sel hepatosit secara abnormal. HCC adalah salah satu kanker dengan prevalensi dan insidensi tertinggi di dunia. Faktor risiko utama HCC di Indonesia adalah infeksi kronik virus hepatitis B, virus hepatitis C dan sirosis hati. Pemilihan modalitas pengobatan yang tepat untuk masing-masing pasien didasarkan pada beberapa karakteristik spesifik pasien, misalnya ukuran tumor, lokasi, trombosis vena porta, serta fungsi hati. Pilihan terapi untuk HCC yang tidak dapat direseksi termasuk intraatrial therapy, multikinase inhibitor, dan imunoterapi. Penentuan stadium menjadi bagian yang penting dalam mengelola HCC karena dapat menentukan terapi yang akan diambil. Salah satu sistem stadium yang digunakan adalah Barcelona Clinic Liver Cancer (BCLC) yang mengkategorikan HCC menjadi 5 stadium (0, A, B, C dan D). Kriteria keparahan klinis dengan stadium BCLC paling sering digunakan karena memiliki validitas cukup baik dalam memprediksi prognosis pasien HCC.
